【明報專訊】Stephen平日生活正常，不煙不酒，偶有運動，前陣子接受例行身體檢查，他自以為一切正常，然而報告竟指他腎臟有事。再作進一步檢查後，原來有「腎水囊」，幸好是良性，醫生說只要觀察，留意水囊有否增生即可。<\/p>\n

通常沒徵狀 出血才疼痛<\/p>\n

腎水囊（Renal Cyst），又稱腎囊腫或腎水瘤，雖然你可能沒聽過，其實頗為普遍。大多數人像Stephen一樣，因為身體檢查而無意中發現，醫生會根據波什尼亞克族分類法（Bosniak classification）把腎水瘤分為四類。有些水囊是良性，大多數毋須理會，不會影響腎功能，亦不像腸道瘜肉可由良性變成惡性；有部分是癌症水囊，要透過手術清除。<\/p>\n

現時醫學界仍未知道腎水囊的成因，究竟是先天因素還是退化所致，尚無定論。這些長在腎上的水囊，小至1厘米，大至超過10厘米亦有；大部分患者的水囊均不會增大，只得小部分人有此變化。通常患者沒有徵狀，然而腎臟長度只有約10至12厘米，如果水囊過大，就會對腎臟形成壓迫，影響腎功能，或者是水囊內出血或發炎，患者才會感到疼痛。不過，因為壓力很低，腎水囊不會如我們想像氣球般爆開。<\/p>\n

水囊太大 手術切開「放水」<\/p>\n

若然水囊太大，評估後可能須以手術處理，醫生以微創手術切除部分水囊牆壁，原理就似椰子開殼，再放水。有患者問，可以不做手術嗎？是否有其他方法取代？有人提出先插針再抽出水分，然而短時間後，水囊又再脹大，也增加細菌感染風險。亦有建議抽走水分後，加入藥水令水囊縮小，現已證實沒用。<\/p>\n

曾有一名60多歲的男士，腎水囊非常大，位於肚子左上方，可以在肚皮摸到，排便也不暢順；最初他向腸胃科求診，擔心是大腸癌，但腸內視鏡檢查後一切正常。經掃描後發現一個12厘米的水囊，與腎臟體積相若。雖然是良性水囊，但因為體積非比尋常，於是以腹腔鏡微創方法消除水囊，此後亦不會翻發。<\/p>\n

除了單一腎水囊，亦有多於一個水囊的情况——多囊性腎病變（Polycystic Kidney Disease），多數水囊較大，成因與基因異常有關，有一定的遺傳性。患者的腎臟因被多個水囊佔據，嚴重者可引起腎衰竭。市民一般的認知是，從電腦掃描看到硬物，才是癌症；但載滿液體的腎水囊，也有可能是惡性，此時的處理方法就要當成癌症來處理，須進行手術治療。<\/p>\n
