【明報專訊】在剛過去的聖誕長假，7歲的Duncan與同學仔希希由家長帶同，一起前來做手術。Duncan的媽媽一早打算讓囝囝割包皮，跟希希媽媽講起，索性兩個孩子同時入院，可以互相解悶。很多男生像他們兩個，天生有包皮過長問題，有些可以翻起，但有些又長又窄，嚴重的可以連小便出口也看不見。小便時，尿液亦容易滯留在包皮內，很易導致尿道炎。由於男性很少出現尿道炎，所以很多個案都與包皮過長有關。由於痕癢，經常想用手搔，或出現發燒徵狀，此時家長就要多關注是否與包皮過長有關了。<\/p>\n

另一方面，由於龜頭不凸出，較易導致龜頭炎，有的可能是閉塞性龜頭炎（Balanitis xerotica obliterans，BXO），徵狀是包皮變硬，這是一種癌前病變，必須進行手術再作觀察。別以為BXO很少見，我不時接觸到這些個案，實在不容掉以輕心。此外，家長亦要顧及兒子長大後的「性行為」，如果龜頭未能外露，儘管不影響生育能力，但由於難以感受刺激，可能會減低行房意欲；有成年男性在發生性行為，始發覺有問題並接受手術。<\/p>\n

割包皮後休息一星期即可<\/p>\n

割包皮即是包皮環切術，以手術方式割去陰莖頭過長或太緊的外皮；部分男童亦需將連繫龜頭與陰莖的繫帶縫好。手術後只需留院一晚，之後休息一星期便可，所以長假期是男童接受手術的好時機。手術後，如廁時尿液可能沾到傷口，為防引起發炎，醫生便會使用「手術膠水」，保護縫線位置，可防水及防菌，比用紗布更衛生。若然男生年紀稍長，睡覺時有勃起情况，或成年男士於早上小便時陰莖變硬，在手術後會處方口服藥物，以減輕勃起，避免令傷口爆線。傷口痊癒後，仍要一段時間才可消腫。由於龜頭外露，與內褲摩擦時可能有不適感，但很快就會習慣。<\/p>\n
