傅﹕前陣子有一名女病人，因為肚痛了兩天加上發燒而來求醫，問過病歷後懷疑是盆腔炎。什麼是盆腔炎？有什麼成因？是否常見？<\/p>\n

柴﹕盆腔炎是指女性的盆腔受到細菌感染，女性盆腔主要的器官有子宮、輸卵管、卵巢及附近組織，如果這些地方出現感染便是盆腔炎。最主要成因是性接觸傳染，令盆腔受感染，嚴重時更可產生膿腫。除了性接觸，曾做過子宮手術或入侵性婦科檢查，都可能導致盆腔炎。另外，有朱古力瘤等婦科疾病，出現盆腔炎的風險也較高。<\/p>\n

■初時腹痛發燒　拖延可致宮外孕<\/p>\n

傅﹕什麼人容易感染盆腔炎？會帶來什麼影響及後遺症呢？<\/p>\n

柴﹕任何有性接觸的女性都有可能感染盆腔炎，尤其是性伴侶較多、性行為沒有用安全套，感染風險相對較高；而曾接受盆腔或子宮手術也容易出現感染。<\/p>\n

盆腔炎的短期影響主要是引起腹痛、發燒、膿腫等不適；若未有及早醫治，可致不育、宮外孕，以及引起長期腹痛。<\/p>\n

■分辨真假盆腔炎 留意陰道分泌或出血<\/p>\n

傅﹕不少病人因肚痛、發燒求診，這些徵狀可以是腸胃炎、尿道、盲腸炎甚至是盆腔炎，如何分辨？盆腔炎有什麼明顯徵狀？<\/p>\n

柴﹕下腹痛、發燒，都是盆腔炎常見病徵，的確與腸胃炎、尿道炎等徵狀相似；但盆腔炎病人要額外注意的徵狀，例如下體多了些帶異味的分泌物，或有不規則出血，即在經期以外有不正常出血。建議一旦有腹痛、發燒，應盡早找醫生檢查，因為盆腔炎若不及時治療，可能會導致不育。<\/p>\n

醫生會為病人做陰道檢查及婦科檢查，若發現異常分泌物，抽取樣本做種菌，以確診是否盆腔炎。另外，婦科及超聲波檢查都可以觀察子宮有沒有膿腫。<\/p>\n

■抗生素可治癒　不易復發<\/p>\n

傅﹕怎樣治療盆腔炎？治療期間有什麼需要注意？會否容易復發？<\/p>\n

柴﹕治療盆腔炎需要使用抗生素，視乎病情嚴重程度，如果是初期及不嚴重個案，可以用口服抗生素，但如出現高燒及持續腹痛，用藥後未見改善，便可能需要入院接受靜脈注射抗生素。<\/p>\n

口服抗生素療程一般是兩星期，療程完成後通常就會康復。治療期間，若病人有腹痛，建議多飲水多休息，用止痛藥紓緩；如有性行為一定要使用安全套，否則可能很快又會再被傳染。病人亦應同時建議伴侶做相關檢查及治療，以免交叉感染。只要處理得當，盆腔炎並不易復發，除非再有不安全的性行為。<\/p>\n

■無關個人衛生　防病要有「一套」<\/p>\n

傅﹕盆腔炎可能與性接觸傳染病有關。提起性病，很多病人有疑問﹕性病是否性接觸才會傳染？使用不乾淨的廁所會否傳染？去泳池、浴室或使用公共毛巾，會否增加感染盆腔炎的風險？<\/p>\n

柴﹕性接觸傳染病只會透過性接觸才感染，想預防就要使用安全套。很多人擔心去了不衛生的公廁、泳池，或使用公共毛巾會感染性病，全屬謬誤，實際上毋須擔心。<\/p>\n
