【明報專訊】夏天,一出門就大汗疊細汗。<\/p>\n
不過,有人天生大汗,站着不動也汗流浹背;相反,有人即使在烈日下走動,滴汗不流。<\/p>\n
西醫指流汗是正常新陳代謝,中醫說流汗太多是體虛表現。汗,十分擾人,汗多汗少原因何在?汗臭如何處理?靜止時、睡眠時流汗,又是什麼原因?<\/p>\n
中醫、西醫拆解汗。<\/p>\n
■西醫說<\/p>\n
「流汗後,以毛巾印乾,別「捽」,怕身體有汗斑或毛囊炎時,連細菌也捽開,散播至其他部位。」皮膚及性病科專科醫生陳厚毅<\/p>\n

問一:炎夏高溫不流汗,是否天生無汗?<\/p>\n
毅︰流汗是身體自然反應,有降溫及排走廢物等作用。流汗多少與遺傳有關,但每個人都有汗腺,都會排汗,女性的汗腺較男性少,加上有些女性較文靜,甚少運動,自然較少出汗,感覺上好像「無汗出」。其實在額頭或皮膚輕微出汗,不過汗水很快隨空氣揮發,因此當事人不覺流汗,身體已啟動散熱降溫功能。出汗少有一個好處,就是毋須擔心體味等問題。<\/p>\n
問二:流汗多,代表有病?<\/p>\n
毅:流汗多可能患上多汗症,患者很容易流汗,就算在靜止的時候也會無故大量出汗,主要是腋下、手掌及腳掌等大汗腺較多的地方。多汗症不會對健康構成重大影響,但卻帶來種種不便。有位16歲少年,因為手掌嚴重出汗,汗水會從手掌滴出來,在考試時,手放在試卷上已有水印,他要不停用毛巾抹汗,未能專心考試,為了應付公開試,故前來求醫。治療方法是在手掌注射肉毒桿菌,阻止流汗信息的傳遞,雖然成功減少出汗,但療效並非永久,大約每半年至九個月便要再注射。<\/p>\n
近年多汗症增兩三成<\/p>\n
臨牀經驗發現,近年多汗症患者人數增加兩至三成,相信與天氣及愈來愈多人愛做運動有關。流汗多的人,較易出現皮膚問題如汗癬、汗斑及毛囊炎等問題,治療方法大多是服藥配以外塗藥物,並不難處理。不過,如果出汗情况突然較之前多便要小心,可能由疾病引發,例如是甲狀腺素高,甚至可能是內分泌腫瘤所引起,最好向醫生求助。<\/p>\n
問三:肥人大汗兼汗特別臭? <\/p>\n
毅:多汗不一定是肥人獨有,多汗症患者也不一定有體臭,當中不少身形高瘦,但以年輕人稍多。有位28歲從事銀行櫃位工作的女士,患上多汗症,腋下更是重災區,產生嚴重體臭,就算已塗了止汗劑及香水亦無補於事,工作時客人隔着玻璃也嗅到她的體味,上司也明言「不如你去冲涼,你會嚇怕客人」,嚴重影響她社交,不敢和同事外出用膳以及謝絕應酬活動,令她毫無自信,見到客人皺眉頭時,她就擔心是否嗅到自己汗味,情緒經常起跌,她也打算辭職。<\/p>\n
微波能量「叮」走汗腺<\/p>\n
後來這女士接受微波熱能除汗(miraDry),原理是以微波能量「叮」走汗腺,屬非侵入式療程,有別傳統割汗腺等手術,只需局部麻醉,而且完成後可即時上班。不過現時美國食品及藥物管理局(FDA)只認可miraDry處理腋下位置。「完成第一次治療後,隔3個月再做第二次,可成功減少82%的汗水。」該女士完成治療後,體味大減,開始與同事有正常的社交,生活回復正常。<\/p>\n

問一:流汗少,健康差?<\/p>\n
升:西醫角度認為流汗少不會影響健康,但對中醫而言,少流汗並非好事。<\/p>\n
身體遇熱流汗,汗可以帶走熱量,相反遇冷時毛孔收縮保溫,用作調節體溫。不過,當氣血運行弱,新陳代謝也較慢,這調節機能會變得遲鈍,對溫度的反應較遲鈍,熱也不會出汗,未能以排汗來降溫,更容易中暑。這情况普遍見於不愛運動,長期在冷氣間,加上少活動及養生,尤其是減肥中的女生,更易「無汗出」。<\/p>\n
有位28歲女士,長期在冷氣辦公室工作,正節食減肥,一個蘋果當作午餐,晚餐只食蔬菜不食飯,新陳代謝慢,營養不足又不夠運動,長期便秘,月經量低,經常頭暈,屬氣虛表現。要改善體質非常容易,只需增加食量,進食適量肉類,多做運動及充足睡眠。<\/p>\n
問二:流得汗多會傷身?<\/p>\n
升:中醫角度,汗是人體的津液之一,更是「血汗同源」,即長期出汗會損心血,可能從氣虛變為血虛,整體也變得虛弱。氣血正常者出汗後可自動調節體溫,但如氣血虛者,大量出汗後會變得虛弱,全身乏力,尤其在烈日下勞動而大量出汗,非常傷身,或會造成「大汗亡陽」,猶如中暑,嚴重者更會昏迷。<\/p>\n
長處高溫 氣血回不到內臟<\/p>\n
不過,就算氣血正常人士,長期流汗亦會影響身體。有位30歲左右的男士,辦公室工作,但後來轉工,每日到戶外工程地盤監察進度,烈日當空下仍要戴上安全帽,汗流浹背。長時間處於高溫下,令血液運行加快,氣血回不到內臟,影響睡眠質素,長期欠缺休息及未能調理身體,一個月後,體質及精神均大跌,經常頭暈、沒精打采,精神不能集中。最後需用一個多月時間調理身體,回復狀態。<\/p>\n
另外,有種汗名為「盜汗」,汗水在睡眠期間偷偷出來。這通常是個人健康狀况不佳,如大病初癒、過度疲勞、長期體力透支及壓力過大時,本身的「衛外之氣」即對抗外邪之氣變得弱時,更易受外邪入侵。<\/p>\n
問三:男人汗,特別臭?<\/p>\n
升:汗味與個人的飲食習慣及工作性質有關,並非以性別決定。如果平日食太多熱毒食物如煎炸、蝦蟹、紅肉及辣味食物等,再加上煙酒,汗味自然較濃,相反食得清淡的人,汗味較淡。另外如果從事戶外工作,汗味也自然較重。從中醫而言,有種汗名為「黃汗」,汗水偏黃色會玷污衣物尤其是腋下位置,更顯眼而且氣味濃郁,通常因出現在脾虛體質,未能運化水氣,故有濕氣困在體內,需以袪濕來止汗。患者不分男女,就算是年輕女士也有可能患上。<\/p>\n
■西醫:衣著貼士:抹汗宜「印」不宜「捽」<\/p>\n
西醫說,首先要保持心情開朗,減少緊張,自然減少汗水;避免穿著太緊身的衣服,選擇通爽的衣料,盡量少去不通風地方。最重要是在未出汗前使用止汗劑,才可成功阻塞汗腺出口,可在出街前或睡覺前使用;出汗後使用就沒有效用。大家每次流汗後,應以毛巾印乾,別「捽」,怕身體有汗斑或毛囊炎時,連細菌也捽開,散播至其他部位。當然最好是冲涼,連油脂也洗走。<\/p>\n
■中醫:飲食貼士:暴曬後 多吃西瓜橙<\/p>\n
中醫說,在烈日下活動,應盡量到陰涼位置降溫及多飲水;另外可以服用人參止汗,人參同時有補氣之用,氣虛人士也宜服用。在暴曬後要多吃消暑食物如西瓜、雪梨、橙、奇異果、白菜、勝瓜、芥菜、西洋菜及霸王花等。<\/p>\n
