「減肚腩,跑步最有效!」註冊物理治療師何淑玲說。<\/p>\n

跑步對消減全身脂肪均有幫助,尤其腹部脂肪先消去,因跑步時,腰腹的核心肌群長時間保持挺直,腹力用得多。但是,中央肥胖人士跑步,豈不增加膝關節負荷,容易傷膝?何淑玲說﹕「中央肥胖人士跑步,的確比一般人易傷膝。腹部脂肪多、向前凸,就如長時間頂着一個大西瓜跑步,令膝蓋承受的力量加重,可能引起前膝痛。」<\/p>\n

不過,這不代表中央肥胖人士不能跑步。「只需事前多做肌肉訓練,強化肌肉,保護關節,並注意跑步時的心跳率,中央肥胖人士都可以跑步減肚腩。此外,最好穿著避震較佳的運動鞋。」<\/p>\n

先做肌肉訓練10分鐘<\/p>\n

何淑玲又舉例,「曾有一個190磅的男病人,有中央肥胖,做了一系列強化肌肉運動,循序漸進地慢跑,每個月減3磅,半年後已減去原先體重的10%,即19磅;最後減至165磅左右,大肚腩亦已收細,可不用再減磅,亦沒有出現膝痛問題」。<\/p>\n

隔天慢跑20分鐘 逐步加操<\/p>\n

她建議,中央肥胖人士可隔天慢跑,一星期三次。「第一至四星期,先做至少10分鐘肌肉訓練,繼而慢跑20分鐘,總運動時間約30分鐘;四星期後,總運動時間可增至45分鐘;六星期後增至1小時。」慢跑時,須保持低強度,心跳率維持在最高心跳率的60%至70%(最高心跳率計算公式為﹕220減去年齡)已足夠,可用心跳帶監察;或可從辛苦程度自行評估,根據辛苦感覺程度(Rating of Perceived Exertion, RPE),若8分為最辛苦,中央肥胖人士跑步適宜處於3至4分的辛苦程度,即一邊跑步,仍可一邊與人聊天。<\/p>\n

初學用跑步機 練跑姿跑速<\/p>\n

針對怕傷膝的人士,何淑玲建議一種名為Pose Method of Running的「慳力跑姿」。她解釋,不少人的跑步姿勢,乃肩、膝、腳跟成一直線,跑時腳跟先着地,腳跟着地的當下,身體向後煞,令之後向前跑時,需動用額外的力度將身體再拉向前。可是,「慳力跑姿」的姿勢是上身輕微向前傾,跑時乃前腳掌先落地,跑步過程中因身體前傾,即借助地心吸力令人順勢踏向前,從而做到「慳力」效果。<\/p>\n

初學跑步時,何淑玲建議使用跑步機,較易掌握正確跑姿及跑速;加上輸送帶的驅動有助帶動身體向前,跑步時較不費力,亦不需要注意路面情况,能專注執正跑姿。直至練習一段時間後,才到街上跑。<\/p>\n

急步行拉筋 待身體復元<\/p>\n

她說,中央肥胖人士減肥,避免太辛苦及急進。「休息也是訓練的一課,不宜每日跑,要讓身體有時間復元,不跑步的日子可拉筋或急步行。」中央肥胖人士多數本身不愛運動,若太辛苦,會很容易放棄,所以慢慢一個月減兩至三磅已不錯,操之過急反而易受傷;同時要注意飲食,多菜少肉、控制食量,否則得不償失。如患者同時有心臟病、糖尿病、高血壓等毛病,運動計劃或需要調整,需就個別情况諮詢物理治療師。<\/p>\n

■四招肌肉訓練<\/p>\n

跑步前必須做肌肉訓練,以防關節受傷,註冊物理治療師何淑玲示範以下簡易四招:<\/p>\n

1. 收腹訓練(V Crunch)<\/p>\n

坐下,雙手抱膝,雙膝貼近胸部、腳掌離地(1a)。繼而收緊腹部肌肉,雙手向橫打開、雙腳抬高和保持離地,全身只以臀部支撐,呈V形(1b)。然後重複抱膝。做10下為一組,重複做3組。訓練﹕腹部肌肉<\/p>\n

2. 平板支撐<\/p>\n

俯卧地上,以前臂及手踭撐起上身,蹬直雙腳,以腳尖支撐身體,收緊腰及臀部,腰部保持挺直。側身看,耳、肩、髖關節可連成一直線。初階維持動作30秒,做2次,若能維持1分鐘,做1次便可。訓練﹕腰腹部核心肌群<\/p>\n

3. 箭步蹲<\/p>\n

左腳踏前,屈膝90度,使身體重心往下壓,右腳在後方呈半跪狀態,屈膝呈90度,繼而雙手伸直向上提高,使腰背保持挺直。維持約5秒後換腳再做,以10次為一組,共做2組。訓練﹕下肢肌肉<\/p>\n

4. 無影櫈<\/p>\n

背靠牆,屈膝凌空坐着,維持15至30秒,做1次。訓練﹕穩定股四頭肌<\/p>\n

■知多啲<\/p>\n

量腰臀比例 比BMI更準<\/p>\n

中央肥胖,俗稱大肚腩,即腹部積聚過多脂肪。不少研究指出,腰圍可反映腹部脂肪積聚的程度,從而評估個人患病風險,標準如下﹕<\/p>\n

男士腰圍90cm或以上(35吋)<\/p>\n

女士腰圍80cm或以上(31吋)<\/p>\n

以往一直用來衡量肥胖程度的BMI,其實未能有效反映身體的脂肪比例。何淑玲建議計算腰臀比例(Waist-hip ratio, WHR),根據世界衛生組織,健康男性腰臀比例上限為0.90;健康女性則為0.85。計算方法是腰圍尺寸除以臀圍,即如腰圍28吋、臀圍35吋,腰臀比例便是28除以35,即0.8。<\/p>\n
